What the question is asking

Item 10 asks whether you have kissed someone while both of you were lying down — on a bed, a couch, the floor, or the grass. "Horizontally" is a euphemism for the position, not for anything else. The item exists because lying down while kissing is treated on this checklist as a step past kissing while standing or sitting, on the reasoning that it usually implies both privacy and time.

Where people disagree about what counts

This is one of the most misread items on the whole list. It does not mean sex, and it does not require that anything beyond kissing happened. The genuine ambiguity is narrower: whether one person lying down and the other leaning over counts, and whether reclining on a sofa is horizontal enough. Most takers read the item as requiring both people to be lying down.

What does "kissed horizontally" mean on the Rice Purity Test?
It means kissing while lying down, as opposed to standing or sitting up. It is about body position only and does not imply that anything further happened.
Does kissing horizontally mean having sex?
No. That is the most common misreading of the item. Intercourse is asked about separately at item 22, and the checklist never uses a euphemism where it has a plain item available.
Does lying on a couch count for item 10?
Most takers count it, since the item is about being horizontal rather than about the furniture. A shared reclining position on a sofa is generally read as qualifying.
